Through this website we hope to improve our services to all our patients and enable you to find the information required at the click of a mouse. You can download registration forms, cancel existing appointments, change your name and address and much more.

You can find out information about medical conditions and diseases through our patient information leaflets as well as keep up to date with all the latest practice news and developments. You can also allow the nurses to collect travel information by using the Travel Form

The person you care for can be a family member, friend or neighbour. Many carers take this role willingly out of a sense of love or committment. Others become carers because they feel that they have no choice. Caring for someone is often rewarding but can be demanding, isolating and stressfull. The centre offers carers impartial confidential help. The centre also offers a home visit service.

Please try to book ahead ... Pre-book apointments can be booked up to 6 weeks in advance. 0844 477 3170 There are a number of appointments available each day for patients who are acutely ill, or, if you do not need seen by a GP or Nurse, ASK for a telephone consultation.

Prescriptions should be collected at one of the local pharmacies.

Prescriptions Request Slips can still be popped into the Surgery prescription boxes in both reception areas.

Routine recall 3 years.

List Cleansing

Please note that each year the Health Board carries out a list cleansing to remove patients that have left the practice.

If you receive a letter from the Health Authority, please make sure that you take the time to respond to their letter.

In the event that no action is taken, you may be removed from our Practice List.
