 September Week-end HOLIDAY ..  CLOSED Friday 16th and Monday 19th September  Re-open Tuesday 20th at usual times.

DROP IN CLINICS FOR FLU VACCINATIONS  will be held on TUESDAY 04/10/16 From 9.00am—7.30pm WEDNESDAY 19/10/16 From 9.00am to 5pm NO APPOINTMENT NECESSARY As this is a DROP IN clinic there will be no guarantee how long you will need to wait. If you are over 65 or have a chronic condition please pop along to have your jab.

Appointments SYSTEM   ...  As from mid-July this will be changing.  When requesting an emergency appointment/housevisit your details will be taken and you will be called back, either by the ON-call Doctor or the Triage Nurse, who will assess your request and decide who is the best person to deal with your emergency.

New Patient Registration.

docandcomputerThe doctors welcome new patients who live within our practice area KA13 postal Codes.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire(see below for over 15yrs & under 15yrs questionnaires) and are offered a consultation with the doctor. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

YOU WILL NEED :   Registration Form & questionnaire completed - HAND QUESTIONNAIRES INTO THE SURGERY WITH THE SIGNED REGISTRATION FORM BEFORE WE CAN COMPLETE YOUR ACCEPTANCE ON TO OUR PRACTICE LIST OF PATIENTS.
