HOUSE VISITS

In February we undertook 203 house visits. This is four times as many as other local practices of a similar size. House visits take a substantial amount of doctor time.

Register with us as a New Patient

If you live within our practice area (KA13 postcodes), you are welcome to register with us and our reception staff will be happy to guide you through the process.

You will need to complete a Registration Form for each individual wishing to register with the practice.

You can complete our online registration form or if you prefer you can download and complete the forms.

Once registered as a new patient to the practice, a ‘New Patient Health Check’ is available with the GP (if you are on repeat medication or have ongoing medical issues) or with the Health Care Assistant if you are not taking medication. Please contact us to arrange a suitable appointment.

Medical treatment is available from the date of registration

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Register online

Step 1: Complete online registration form

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Step 2: Upload scans of photo identity, and proof of address

Upload scans of photo identity, proof of address and completed New Patient Questionnaires as appropriate (links below). One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

Please download and complete ALL the required forms, failure to do so will delay your registration with the Practice. If you are unable to download the forms then please contact the surgery for advice on how to register with us.

2: New Patient Registration Questionnaires

As it often takes some time for records  to be forwarded from your previous practice you will also need to complete a New Patient Registration Questionnaire for each person registering – there are separate forms for patients 15 years and over and patients under 15 years of age.

Once completed please upload the forms to the practice, by accessing the button shown below.

If you prefer please bring the completed forms along with you when you come to the Practice to register and provide us with identification and proof of address.
